ACS - Alabaster City Schools will host free flu shot clinics for students at all ACS schools on Wednesday, October 21.
The clinics will begin at 8 a.m. and continue throughout the school day. Each school will announce when students should report to the clinic.
The clinics are free for enrolled students, with the exception of students covered by PEEHIP insurance. PEEHIP members will have access to a separate flu clinic later in October.
Parents and guardians must complete the official flu shot consent form for their student to participate. The form is available on the ACS website in English and Spanish.
Students should bring a printed, completed consent form to school on October 21. Families who are unable to print the form may email the completed form to their child’s school nurse, who can print it before the clinic.
Families are encouraged to complete the consent form in advance to help ensure a smooth process on clinic day.
